Just realized I put my review in the wrong place😬 Oh well.. I have had 3 stays at Wolf Creek over the last year for different issues, and each time I have received very good care. From the CNA group Crystal (spikey hair), Curtis, Santana, Jasmine, Doka, Uly and just everyone there made the days bearable and were very helpful and caring. The PT group Brandon, Alexa, and Meredith thank you to each of you for your care. Vicki McDaniel a big thanks to you for your knowledge and expertise! The best part is all the laughs, hard to come by in this situation! I know I left people out, but want to thank each and everyone of you for your exceptional care! Hopefully I won’t be back unless to say hello🤗 Oh, and last but not least thanks for tracking down my PJ’s.

Best care ever! The staff is friendly and helpful! 2nd time my mother has been here in 2 years. The nurses and CNA’s, Therapist, administrator, admissions, social services , Dr. Prichett, and all the aides! Cannot tell you how much we appreciate the care she received, to coordinating her coming home on hospice care! Thank you!

The reason I gave them two stars is because the staff is absolutely wonderfu,l every single one of them was absolutely wonderful, they’re always smiling, they’re always happy to help and they do a fantastic job. The reason I didn’t give them five stars is because, four people have to share one bathroom absolutely horrid that’s four people using one bathroom and believe me nobody goes in and cleans as soon as one person comes out, so if you don’t mind sharing your bathroom with four strangers then I guess it won’t bother you. The nursing and people who take care of you are the best and most wonderful people, however I can’t say the same for the higher up staff the people you don’t see the people behind the closed doors, they’re the ones I wouldn’t give one star to.l and the food is terrible.

My mom was here for 2 months and I cannot say enough about the kindness of the CNA’s and especially the physical and occupational therapy teams. HUGE shout out to Brandon and Jacqueline and Vickie in social services.

Leadership & Planning: Vicki McDaniel was nothing short of a miracle worker. Her guidance in crafting a long-term home care plan was thoughtful, strategic, and deeply personal. She didn’t just help us plan, she empowered our family to move forward with clarity and confidence. Nursing Team: Led by Sher, the nursing staff, Jackson, Angel, Jasmine, Haley, and others, delivered top-tier care with exceptional bedside manner. Their blend of clinical expertise and human warmth made a difficult time feel manageable and dignified. CNA Crew: Santana, Vanessa, Crystal, Curtis, and their colleagues brought energy, kindness, and consistency to every interaction. They treated my father not just as a patient, but as a person, with respect, humor, and genuine care. Therapists: Occupational therapists Alexa and Meredith, along with physical therapist Anandkumar, were phenomenal. Their tailored approach to therapy helped my father regain strength and confidence, while always honoring his unique needs as an ALS patient. Support Staff & Dining: Even the food exceeded expectations, thanks to Esteignha and the culinary team. The menu was thoughtful, nourishing, and surprisingly delicious, adding comfort to every day. Final Thoughts: Wolf Creek doesn’t just provide care, they embody it. Every team member, from leadership to support staff, treated my father as if he were their own. If you’re seeking a facility that blends clinical excellence with heartfelt compassion, Wolf Creek is the gold standard.

Wolf creek is a great skilled nursing facility with caring and empathetic staff. I highly encourage anyone who is looking for skilled nursing care to check them out!

I stayed at Wolf Creek for about 3 1/2 weeks after getting lassoed with my dog’s cable line and breaking my femur. I can’t say enough wonderful things about my time there. All of the staff were warm, kind and helpful. From the top administrator to the doctor, nurses, CNA’s, kitchen staff, PT and OT staff, social services and cleaning staff. Seriously everyone was amazing. I developed such a lovely rapport with each and every one of them. As happy as I am to be home, I couldn’t have asked for a better experience in a skilled nursing facility. If I ever need one again, I will definitely go back there!

I transferred after two weeks in their sister facility and immediately felt a more positive atmosphere. I put on weight I had lost in the previous weeks and improved dramatically with their encouragement- based OT and PT programs. I was truly well when I was discharged.

My stay at Wolf Creek Rebab facility was extremely pleasant more than I thought would have been. The staff was very friendly, caring and empathetic to my needs. My roommate during my stay was the sweetest little lady. She always kept me on my toes, and we shared a lot of laughs. She was always sweet friendly, willing to help me when she could. There’s not a complaint to be had regarding the nurses and staff. All were very informative and kind and over the top willing to make my stay comfortable. The only thing I would suggest is to possibly get standing heated food try holders. The food was often cold. Not a complaint just a suggestion. I would recommend your facility to everyone. Thanks for the good care and continued care since being discharged. I am on my way to recovery, and am looking forward to a full recovery. In 2023, my father received a diagnosis of stage IV cancer, necessitating full-time care. Residing in Southern California while I was in Nevada County, I faced the challenge of relocating him closer. Through the invaluable assistance of Samantha Phillips, a dedicated professional aiding families in such transitions, (now a nurse at Wolf Creek Care Center) we secured a place for him at Wolf Creek Care Center in Grass Valley. The care he received there was nothing short of exceptional. At Wolf Creek, the compassionate attention from the CNAs and nurses was complemented by the heartfelt dedication of social worker Vickie and activity director Debbie. These two remarkable individuals not only fulfilled their professional roles but also became cherished friends to my father. Embracing his new community, he took on the role of president of the residents’ council, actively participating in monthly meetings to enhance the quality of care. He held the entire staff in high esteem and affection. Wolf Creek Care Center became more than just a facility; it was truly his home. Given his inability to walk and my limitations in providing the necessary care, it was a relief to know he was surrounded by a team that treated him like family. Their unwavering support and kindness have left an indelible mark on our hearts. For families seeking a nurturing environment where their loved ones will be treated with the utmost care and respect, I wholeheartedly recommend Wolf Creek Care Center. Following my father’s passing on January 16, 2025, the staff graciously invited me to join their team as a volunteer. Now, I will have the honor of visiting and supporting the residents they so diligently care for, continuing the legacy of love and community that my father cherished. With all my love I thank every single person who works at Wolf Creek Care Center. You went above and beyond to make my dad’s last two years of his life memorable for both of us.

I am a friend or relative of a current/past resident I have had incredible peace of mind since my sister has been in Wolf Creek. My sister is amazingly well cared for, happy and her health is improving. My sister has been in multiple skilled nursing care facilities since 2020 including board and care facilities. It is hard to find good facilities. The quality of care at Wolf Creek is exceptional and impeccable. The staff is caring and quickly responsive and proactive in managing her health issues with the best CNA’S, Nursing staff, physical therapists, activity directors, management and especially social services. I can not recommend this facility highly enough.

My family members stay here has been very long and the staff is very accommodating and on point. The Staff is incredible and very interactive with all the guests here not just my loved one. Wolf Creek is truly an extraordinary place.

I pray for my cousin Todd gillogley as he is dealing with his health I have great confidence and faith in your team there that he will get better. All things are possible through christ and healing. While a few of the staff are pleasant, there is an overall lack of communication between staff when it comes to patient care. Additionally, there is an unsatisfactory wait time for patients using their call buttons to request assistance. Would not recommend this facility if one has other options.

This is a wonderful facility. I am deeply grateful for how our son was taken care of while he was here. I highly recommend it.

This place was amazing. Staff was beyond caring and respectful as my dad passed. They allowed me to visit nearly anytime bring anything he wanted. Was a great feeling to have they help we did from wolf creek at such sudden notice. Thank you wolf creek staff!!!

My husband was there for about 4 weeks. He got food poisoning 2 times and they tried to sweep it under the carpet. Horrible place.

I cannot recommend this skilled nursing home enough. The staff is compassionate and dedicated, the facilities are top-notch, and the care provided is truly exceptional. My loved one has been a resident here for several months and the difference in their health and overall well-being is astounding. From the moment you walk in, you can tell that the staff truly cares about the residents and goes above and beyond to make them comfortable. The therapy and rehabilitation services are also top-notch. The meals are delicious and always served with a smile. Overall, a fantastic skilled nursing home that truly cares for their residents.

I visited this facility The staff at Wolf Creek Care Center was very pleasant and very helpful. They pretty much did everything they could to see if my husband could go there. They showed me the exercise facility, dining room, crafts area, and walking space in the hallways for residents to move about. It was very clean. The negative part was that it’s not an Alzheimer’s secured facility.
